Build Your Event Registration Form

With RSVPify's Form Builder, you can create a registration experience that's as simple or as detailed as your event requires. In this guide, you'll learn how to build your form, organize your questions, and prepare your registration experience for launch.

Your registration form is where guests tell you they're coming and provide the information you need to plan a successful event.

Whether you're collecting RSVPs for a private dinner, registrations for a conference, or ticket sales for a fundraiser, your form becomes the foundation of your guest list. Every response helps you make planning decisions, from finalizing catering counts to preparing name badges.

Before you start

Before building your form, think about the information you'll actually need to run your event. Most registration forms collect:

  • Full name

  • Email address

  • Attendance response or ticket selection

  • Meal preferences

  • Additional guest information

  • Accessibility or dietary accommodations

Keep your form as short as possible. Every question should help you make a planning decision - if you won't use the answer, consider leaving the question out.

Understand the Form Builder

Your registration form is organized into four main areas:

Guest List Lookup

Allows invited guests to locate their invitation before registering. This is commonly used for invite-only events.

Pre-registration

Appears before the registration form and is useful for sharing instructions or important information before guests begin.

Questions

This is where you'll build your registration experience by adding the questions and fields guests will complete.

Just like the Website Builder, your form is made up of sections and blocks. Sections help organize your form into meaningful areas, and each section can contain one or more content blocks.

To edit any block, click on it in the left-hand panel - the settings menu for that block will open on the right side of the screen, where you can customize its content, appearance, and behavior.

Confirmation

Controls what guests see after submitting their registration, including confirmation messaging and next steps.

Start with the basics

Every RSVPify registration begins by collecting the primary guest's contact information.

Email address is required because RSVPify uses it as each guest's unique identifier. This allows RSVPify to:

  • Create and update guest records

  • Send confirmation and event emails

  • Track registrations and RSVP status

  • Manage guest communications

  • Support check-in and attendance tracking

Every registration includes:

  • Full Name

  • Email Address

From there, guests continue through the rest of your registration experience.

Build your registration flow

After collecting contact information, think about the order your guests should complete the form. A typical registration flow looks like:

  1. Contact Information

  2. RSVP or Ticket Selection

  3. Event Questions

  4. Additional Guests (if applicable)

  5. Calendar Invitation

  6. Review and Submit

Organizing questions in a logical order creates a smoother experience and makes forms easier to complete.

Add the questions you need

Select Add a Question to browse the available field types. Common question types include:

  • RSVP responses

  • Ticketing

  • Meal preferences

  • Multiple choice

  • Dropdowns

  • Checkboxes

  • Donations

  • Open-ended text responses

  • Terms & Conditions

  • File uploads

Choose the field type that best matches the information you need to collect.

Collect event-specific details

Once you've gathered the basics, add any additional questions needed to plan your event. Common examples include:

  • Dietary restrictions

  • Accessibility accommodations

  • Company or organization

  • Job title

  • Session selections

  • Seating preferences

  • T-shirt sizes

  • Emergency contacts


Only ask questions that will help you plan the event or improve the guest experience.

Register additional guests

IIf your event allows guests to bring additional attendees, you can enable the option for registering extra guests. This lets the primary registrant submit details for multiple people in one go - perfect for plus-ones, families, or small groups.

For public events, you can easily control the number of additional guests directly within the registration form by setting limits (none, capped, or unlimited).

Set up a calendar invitation

Add a calendar invitation step so guests can save your event directly to their calendar after registering. This helps reduce no-shows and keeps your event top of mind.

Match your branding

Use the Theme tab to customize the appearance of your registration form. Apply your organization's:

  • Colors

  • Fonts

  • Button styles

Consistent branding creates a seamless experience between your event website, registration form, and guest communications.

Personalize the experience with logic

Not every guest needs to answer every question. Use the Logic tab to show questions only when they're relevant. For example:

  • Show meal selections only if the guest is attending.

  • Display sponsor questions only for sponsor registrations.

  • Ask accessibility questions only when accommodations are requested.

Using logic keeps forms shorter while still collecting the information you need.

Preview before you publish

Before opening registration, complete a full test registration. Recommended checklist:

  • Preview the form

  • Test desktop and mobile layouts

  • Complete a test registration

  • Verify required questions

  • Test any conditional logic

  • Review the confirmation page

  • Confirm the guest appears correctly in your guest list

Testing before launch helps ensure a smooth experience for your attendees.

Publish and update your form

Once your registration form is ready, publish it so guests can begin registering. As your event evolves, you can continue updating your form by:

  • Adding or removing questions

  • Updating ticket options

  • Revising instructions

  • Adjusting logic

  • Updating confirmation messaging

Once you've made changes, republish your form so future guests see the latest version.
